Configure default dates in standalone digitisation form

Currently the standalone digitisation form prepopulates the start and end date for a request as today's and tomorrow's date respectively. This will never reflect the actual dates required and creates confusion. Ideally we would like to be able to set these dates on the admin side and prevent requesters editing them. Failing that, we would like the date fields to all be blank to avoid some confusion.
