The purpose of the report is to produce data on reporting Service Level Agreements, as well as finding out averages, etc.
It would be really helpful to have further information on how you see this report working, are you looking for a worklog report for each requests or something at a higher level? More information may allow others to input to the idea and vote on this.

It would helpful to know who has been working on a request, who completed a scan, who mediated checks etc to help us understand the impact on staff time to identify how to manage staff in relation to other projects/tasks etc
